Ultrasound Consent
We now offer an ultrasound system that shows a live video broadcast for spouses that an unable to come to the appointment. For legal reasons we need any person that is going to remotely view the ultrasound to sign a wavier for Ultrasound Consent. Please print, sign, and send a copy of it to us if you would like to take advantage of this service.
